Dr. Komal Balaney, Dr. Sailaja Gadde and Dr. Reshma Shah in our Suwanee/Johns Creek office have recently been appointed Civil Surgeons by the U.S. Citizenship and Immigration service. This means they are each authorized to perform the physical examination required to complete the Report of Medical Examination and Vaccination Record (I-693 Form). If you need this service performed please call our Scheduling Department to have them set up an appointment for an Immigation Physical.

Before coming to your appointment, download the I-693 form from the U.S. Citizenship and Immigration service website and fill out Part 1 completely. Please note a separate I-693 form and examination is required for each applicant. When coming to your appointment each applicant must also present a government issued photo identification. For applicants under 14 years of age other proof of identity that shows name, date and place of birth and parent's full names is acceptable.
